I have an IM and had initially planned a home birth but it is not to be. However I want to get my midwife something special to say thank you to her. She has bent over backwards for me throughout this pregnancy.

No gift. A thank you card but nothing more. They are being paid.

Sorry, just realised this is the HB section, sorry.
I have however always given a small gift to the midwives who have assisted me in labour and would imagine that if I'd formed a close relationship as you would with an IM I would put quite some thought into an appropriate gift.
I like GF's ideas.
There is never an expectation of a gift but I have bought gifts for many many caregivers because I felt they deserved it...despite the fact they were paid.

Usually nothing as they are being paid. Chocolates (or wine) are always good.
I guess being an IM it's a bit different as she's doing all your care (are you having a home birth). But she won't be expecting a present. I card is always great with a photo of your baby. You can always send updated photos for a little while too.
One thing to remember is that most people only give presents to the midwife at the delivery but then they may get exceptional care from all the midwives on the postnatal ward but not acknowledge them (that's where a group thankyou ie box of chocolates) is always appreciated.

Hmmm, nothing at all! Not even a card. I'm not really that organised. I did have croissants in the freezer to serve for breakfast after the birth .... not sure how I knew that the baby would be born at 7am but she was, so we had croissants and jam ..... that was the extent of it!

That was really good idea gifting your midwife with a gift.
Along with a greeting card, give some sweets or bag.
