That just all sounds horrendous OP. I second the good physio for helping manage pain and back issues. I get what you are saying about lack of funds but I swear by mine, the only reason I'm still mobile from disc issues post birth is my physio and her amazing care. Try and see if you have a sports physio that also knows about post birth issues  in your area, someone that is all about getting you moving.

I’m so sorry you are in such pain and have no answers.

I think it might also be worth chatting with a breastfeeding and medication specialist to discuss the options for pain relief.

Have you seen your GP for a pain management plan? They have an actual name, where you get subsidised sessions with two different providers to help with health issues?

I had a big 2nd baby and similar pain. Dr sent a referral to the community physio (which is free), so don't rule it out as it may be able to be done under the public system.

I agree the shooting pains sound like sciatica to me. They can be very painful IME. Have you seen a physcio?

Is there a community health service near you? They usually have physios and generally at a much lower cost than your usual high street physio practice.

And/or perhaps call the hospital you delivered at and ask what physios they recommend for women’s health? Or ask if you might be able to be seen by their in-house service?

Gosh that's not been well managed. I am a physio and unfortunately that doesn't surprise me.
Find a good physio. Back pain takes time to recover from- find some one who can tell you where the pain is coming from and teach you how to manage it And get stronger.
I see women develop pretty debilitating back pain after having a baby very often. It's a lot to do with the weakness of muscles around the hips, pelvis and tummy and all the demands of looking after a slowly increasing in size bub that you're bending over cots awkwardly, or picking up from the ground.
Occasionally of course it's not actually the back, but you've had a lot of investigations there too. Even if a Neuro surg finds a slight disc herniation they usually want you to manage with physio first.

OP, since you've had this for 17 months I'm wondering whether you would qualify for a chronic disease management plan to get 5 subsidsised visits with a physio?

Or, since this was pregnancy related, can you contact the hospital where you had your baby and see if they have a physio that you can see?

I have both a FAI and a spondylolisthesis and I get this kind of pain regularly.

I would seek a women’s physio and get some Pilates I find it helps as the lower back is Taking the bulk of the work the core muscles are supposed to be doing and it’s referred down the buttock and leg. I find Pilates and myotherapy helps me.

My spondylitis was found via a simple X-ray.

I know you mentioned you are still breast feeding, but given the impact this is having on your life (and your child's) I wonder if you would consider weaning, so you have more drug options available to you. Obviously you need to continue to pursue a diagnosis as well, but you need to be able to live and have some enjoyment from your life.
You may not want to consider this, but thought it was worth mentioning as an option. Good luck finding the cause, and hopefully some relief, from your pain.
