A question about poo.......

Sorry ladies, but I'm not sure where else to ask.
My DS is 5 months old, fully forumula fed and sometimes (most of the time) his poo is soft and watery. It's hard to explain, but it's like it has hard bits in it - not hard but toothpastey but then watery stuff comes out too.
A couple of times he has poo'd during nudey time on his rug and it's like there is the pastey stuff, but then a ring of discoulered area on the rug where the watery stuff has soaked in. I've noticed it in his cloth nappies also.
Often if he is in a disposable, you can see where the watery liquid has soaked the disposable only leaving some muchy bits on top.
He is not a skinny baby by any means and has just started now to slow down weight gain. He hasn't gained any weight in the last 3 weeks but the last nurse I saw said that is normal and so long as he is following the same growth curve (he is - 75th percentile) than it isn't a problem.
I often struggle to feed him his bottles during the day. He doesn't seem very hungry or starts hungry but then pushes the bottle away. He will struggle to have 3 x 150ml during the day but then seems ravenous at his last feed of the day (6.30pm ish) and drains 240ml.... He wakes 2-3 times a night and can be quite difficult to settle (for his day sleeps also)
He has been on the same formula since he was about 5 weeks old.
I have an appointment with the nurse tomorrow but was wondering if anyone else has experienced this.

The only thing I can think of is that he is wee'ing in his nappy when he poos to cause the watery consistency. Possibly the same on the rug.
Have you tried dropping him down to 120mls and giving him 5 bottles a day and not 4?
What time is the first time he wakes during the night? If it is before midnight, have you tried giving him a 5th bottle then?
His weight doesn't seem and issue and you haven't mentioned that he screams or throws up often, so I personally don't think there is anything sinister going on.
Does that help at all? All the best for your apt tomorrow.

He gets 5 bottles a day now. 3 x 150ml, 1 x 240ml and then another 150ml when he wakes in the night. I am currently only feeding him once during the night but settling him the other times. He used to only wake once during the night.
The last two nights he woke at 11.30pm and 4am. the first night I fed him at 11.30pm and then re-settled at 4am. Last night I re-settled at 11.30pm and fed at 4am. Maybe I should just feed him both times??
I thought maybe he is starting to wake up hungry indicating solids may be soon but he doesn't seem at all hungry during the day and I tried him on some rice cereal and he wasn't very fussed. who knows.
But you're right - he isn't screaming or fussing at the bottle - just doesn't seem interested. He has started to posset alot more than usual. Not vomit, but just possets of curdled milk.
Maybe it's teething? Who knows, it's such a guessing game. I just hate the thought that something might be wrong and that is why his sleep is so bad.
