My nearly 4yo has had temperatures since Friday night. He has been groggy and sleepy, and on constant panadol since Saturday morning. He reacts well to the panadol, but as soon as it wears off, he gets the major sweats, is shaky and delirious, so we top him up again.

We took him up to the gp clinic at the hospital yesterday afternoon, an hour after he took panadol so his temperature was down to 37c and he was smiley and talking, so they sent us on our way (understandably) but is still the same today, and is now refusing to eat more than a biscuit because it makes him gag and throw up bile.

I just temped him just before his last dose of panadol and it read 40.1c (under arm temp) and then a hour after panadol it read 38.5c.
I am hesitant to take him back to the clinic because they think it's best to ride it out, they believe it's viral. I understand that, and am definitely not a hypochondriac mother or panicker, but I feel with those kind of temps over nearly 4 days now just seems like a lot.

Is he drinking and passing urine? My biggest concern would be poor fluid intake. If not drinking and hasn't improved today then I would head back to GP.

TBH, I'd be taking him to the hospital. They always tell me to bring DS in if he's had a fever of over 38.5 for 3 consecutive days.....

He isn't passing as much urine as usual, but he isn't drinking as much as usual. Maybe 3-4 sips an hour, I try but can't force him to drink any more.

They did a urine sample in the clinic yesterday and they said it was fine, so up until that point he mustn't have been dehydrated.

I would probably go to hospital to, as 40.1 underarm is very high considering you should add 0.5-1 to an underarm temp. Given the fever has been happening for days i would be wondering if there is an infection that needs antibiotic.
